ABSTRACT

Liver cancer is one of the most common malignant tumors at present and has high incidence and mortality rates. Most patients are in the advanced stage at the time of diagnosis and thus lose the opportunity for surgery and have poor prognosis. At this time, medical treatment focusing on drug therapy becomes an important method for the treatment of advanced liver cancer, including chemotherapy, targeted therapy, immunotherapy, traditional Chinese medicine treatment, and endocrine therapy. This article reviews the recent research advances in drug therapy for advanced liver cancer.

ABSTRACT

Objective To evaluate an effective and feasible quantitative evaluation table of traditional Chinese medicine (TCM) syndrome differentiation, and to observe the effect of combination of TCM syndrome differentiation and standard bundle therapy in patients with septic shock. Methods A prospective randomized controlled trial was conducted. The septic shock patients with acute deficiency syndrome admitted to department of critical care medicine of Ruijin Hospital, Shanghai Jiao Tong University School of Medicine from January 1st, 2016 to December 31st, 2017 were enrolled. The patients were randomly divided into control group and Shenfu group. The patients in both groups received early application of standardized bundle therapy; those in Shenfu group received 60 mL Shenfu injection infusion in addition for 7 days. The TCM syndrome score was evaluated by classification and scoring method of TCM symptoms. The circulation and tissue perfusion, severity of disease, organ function, inflammation response, adjuvant treatment and 28-day mortality were compared between the two groups. Results A total of 50 patients with septic shock were enrolled in the analysis, 25 in control group and 25 in Shenfu group. The markedly effective rate of TCM symptoms score in Shenfu group was significantly higher than that in control group [60.0% (15/25) vs. 16.0% (4/25), P < 0.01]. There was no significant difference in all parameters before treatment between the two groups. After treatment, the observation indexes of both groups were improved. Compared with control group, the mean arterial pressure (MAP) in Shenfu group increased more significantly [mmHg (1 mmHg = 0.133 kPa): 13.0 (2.5, 28.5) vs. 6.0 (0, 13.5)], the lactate (Lac) and procalcitonin (PCT) decreased more significantly [Lac (mmol/L): 0.8 (0.1, 3.7) vs. 0.5 (-0.6, 1.7), PCT (μg/L): 2.0 (0.7, 32.3) vs. 0 (-1.8, 3.8)], activated partial thromboplastin time (APTT) was shortened more significantly [s: 8.5 (0, 12.9) vs. 0 (-7.2, 10.0)], and interleukins (IL-2 receptor and IL-6) levels decreased more significantly [IL-2 receptor (ng/L):1 031.0 (533.0, 1 840.0) vs. 525.5 (186.0, 1 166.8), IL-6 (ng/L): 153.1 (21.4, 406.8) vs. 35.1 (16.3, 110.1)] with significant differences (all P < 0.05). There was no significant difference in the use time of vasoactive drugs, duration of mechanical ventilation, severity of the disease or 28-day mortality between the two groups. However, the use time of vasoactive drugs in Shenfu group was shorter than that in control group (days: 5.48±4.81 vs. 8.28±7.83), and the 28-day mortality was decreased [8.0% (2/25) vs. 20.0% (5/25)]. Conclusions TCM syndrome score is helpful to evaluate the effect of TCM syndrome differentiation and treatment, and it is effective and feasible in clinical application. Septic shock patients treated with TCM syndrome differentiation and treatment combined with standard bundle therapy were significantly improved in circulation, tissue perfusion, coagulation function and inflammation reaction.

ABSTRACT

Objective: To detect the expression of P53, human epidermal growth factor receptor-2 (HER-2), and tumor endothelial marker 1 (TEM1) in gastric cancer tissues, analyze their correlation with clinical efficacy, and explore their potential roles as biomarkers for neoadjuvant chemotherapy. Methods: Sixty-three patients with gastric cancer who underwent fluorouracil-based neoadjuvant che-motherapy in The First Hospital of Lanzhou University from May 2015 to May 2017 were enrolled. Using immunohistochemistry, the expression of P53, Her2, and TEM1 was detected in 63 gastric cancer specimens before neoadjuvant chemotherapy. The efficacy of neoadjuvant chemotherapy was assessed by imaging. The relationship between the expression of P53, HER-2, and TEM1 and the effi-cacy of neoadjuvant chemotherapy was analyzed. Results: The total effective rate of neoadjuvant chemotherapy in 63 patients with advanced gastric cancer was 69.8%, with 2, 7, and 35 patients achieving complete remission, partial remission, and stable disease, re-spectively. Disease progression was noted in 19 patients. Univariate analysis revealed that patients positive for TEM1 and having high T stage had a poor response to neoadjuvant chemotherapy (P<0.05); furthermore, location, differentiation, and size of tumor; P53 posi-tivity (P=0.488); and Her-2 positivity (P=0.106) were not associated with the efficacy of neoadjuvant chemotherapy for gastric cancer. Multivariate analysis revealed that TEM1 positivity and a higher T stage could be factors that predicted the response to neoadjuvant chemotherapy in patients with advanced gastric cancer. Conclusions: TEM1, as a marker of tumor stroma, may be an important molec-ular biological indicator that predicts the poor response to neoadjuvant chemotherapy in patients with gastric cancer.

ABSTRACT

Progress in the treatment of gastric cancer(GC)is a major concern because GC is a common cancer worldwide.Recently,by studying the molecular mechanisms of GC,researchers learned that GC progression involved a series of changes at the molecular lev-el,including the activation of signaling pathways, abnormal expression and mutation of genes, and response to related targeted drugs used in clinics. However, many targeted drugs produce resistance in a short period of time because of complicated cellular,microenvi-ronmental,and genetic instabilities. Therefore,it is important to find new targets against GC.In this review,we present the current re-search status and progress with respect to GC targets.

ABSTRACT

To assess the association between hypoxic inducible factor-2alpha [HIF- alpha] and hepatocellular carcinoma [HCC] by meta-analysis. This study was carried out at Anhui Province Hospital, Hefei, Anhui, China in February 2014. We searched various databases for studies published in English or Chinese up to February 28, 2014. The hazard ratio for overall survival and analyzed odds ratio were combined to evaluate the clinicopathological features of HIF-2 alpha expression in HCC. A total of 7 eligible studies comprising 1066 patients with HCC were identified after our full assessment according to inclusion criteria. All of the patients came from China. The results indicated that the association between HIF-2 alpha expression and prognostic values in HCC was inconspicuous, while the expression of HIF-2 alpha was significantly associated with capsule infiltration, vein invasion, and histological grade. Expression of HIF-2 alpha was associated with invasion and metastasis in HCC, but did not have a distinct significance in prognosis, according to the limited evidence. However, high quality, large sample size, and controlled trials are required

ABSTRACT

Objective To generate the erythroid differentiation associated gene(EDAG) knockout mice and analyze their sensitivity to low dose radiation-induced damage. Methods Zinc finger nuclease technology ( ZFNs ) was used to produce the EDAG knockout mice. The low dose radiation-induced damage was evaluated by peripheral blood cell counts, DNA damage and colony formation of bone marrow cells.Wild-type and EDAG knockout mice were irradiated with 0.31 Gy/min X-ray, one minute per day for seven consecutive days, and the cumulative radiation dose was 2.17 Gy(n=7).The blood cell counts were measured by an automated hemocytometer.DNA damage was detected by immunofluorescence assay with a DNA damage marker p-H2A.x antibody (n=3).The colony formation ability of bone marrow cells was evaluated with a semi-solid culture medium(n=3).Results A model of EDAG knockout mice was established.Compared to wide type mice, white blood cell counts of EDAG knockout mice decreased significantly while the DNA damage marker p-H2A.x expression was increased on the third day after X-ray irradiation. The ability of colony-forming was reduced after 7 days of X-ray irradiation. Conclusion Our present study found that EDAG knockout mice are more sensitive to low dose radiation-induced damage as shown by decreased peripheral blood cells counts, reduced colony-forming ability of bone marrow cells, and increased DNA damage. These results suggest that EDAG knockout mice can serve as a powerful tool for evaluation of the biological effects of low-dose radiation damage.
